Sly Spy is mostly a side-scrolling platform shooter that also including scenes of parachute combat, shooting while on a motorcycle and underwater combat. All of these are made to fit the game's '60s-'80s spy aesthetic made famous by James Bond. The game allows you to enter your own agent number (which becomes your high score initials) and players who don't finish entering the numbers before the timer runs down are automatically christened as 0-0-7. Many more bits of Bond lore are referenced, including the Golden Gun (used as a power-up weapon), Oddjob (more on him later) and Jaws. Just as the Bad Dudes rescued President Ronnie, the imperilled president in this game seems to be based on George H.W. Bush, as seen in the attract mode cinema.

The game is filled with shout outs to other Data East titles, including posters with Karnov and Chelnov, signs with the U.S. Bad Dudes logo and what appears to be the headless corpse of RoboCop (Data East made the official arcade game one year prior to this) in the last stage. The game's fifth boss, who is based on Odd Job of the Bond series, also seems to be one of the more secretive Karnov cameos, because it's rarely mentioned in any site talking about this game. The boss has the basic shape and fighting stance Karnov had in Bad Dudes, but he has two strips of hair and a slightly thinner waist line. Perhaps he's just a relative.

The game itself, under the Sly Spy name, appears in the film "RoboCop 2" when Murphy and Lewis have a run-in with bad guys in an arcade which just so happens to be heavily stocked with highly-visible Data East games. RoboCop throws a bad guy head first into the Sly Spy cabinet's screen.
